I've seen what I think to be this same issue.
In the ATA--> Style & Edit Header Area --> Configure Header Area...
Make sure there is only one reference to %image in that field.

Sorry, I meant to include the URL:
http://www.ouryogablog.com
I tried the configuration area, it defaults with %pages %logo %bar1 %image %bar2
I removed the last 2, but it made no difference

go to ato->export/import setting and export your settings and attach them to a reply (use the paper clip icon)

You ha ve
HTML Code:
%pages %logo %bar1 %image %bar2
in your "configure Header Area' option
The %logo will display the image you have specified in the 'Show Logo Image?'
the %image will display the images in the atahualpa/images/header folder.
Now I see you have copied the 'header6.jpg' to your images folder and have chosen it as the Logo Image. This explains why you are seeing it twice.

OK, so in the Style & Edit Configure Area it says this:
To show your own graphic, upload an image to /wp-content/themes/atahualpa/images/ and put the file name of the image into this field.
I thought it was pulling this graphic from the /images/header area where I have just the one file header6.jpg.
I just removed the same file from the /images folder and that appears to have taken care of the duplicate header issue.
But I still do not see why it isn't finding my custom graphic.

1. If you want your custom logo to appear.
Upload your logo image in /images and put the filename in the field. And delete the %image from configure header area.
2. If you have some custom headers
upload your custom headers in /images/header and delete the original atahualpa headers, your headers gonna rotate (if you have more than one) or stay still if you have only one. And delete the %logo from configure header area.

You CAN have both %logo and %image
%logo will display the logo area - the logo image goes in 'atahualpa/images' folder
%images will display the header image area - it gets these images from the 'atahualpa/images/header' folder
Think LOGO and HEADER images.

Still lost. I deleted %image from the configuration and it lost the header totally. So I put it back but the header hasn't returned.
I have header6.jpg in both folders. Now what?

Why do you have it in both folders?
If you have a logo image,
1) put it in the images folder and call it 'my-logo.jpg'
2) If you want to see the logo, put 'my-logo.jpg' in the option 'ato->Style & edit HEADER AREA->Show Logo Image?' and add %logo to the 'ato->Style & edit HEADER AREA->Configure Header Area' option.
If you don't want to see the logo, don't use %logo.

I apologize, but this still is behaving badly for me.
This is what I have in the settings;
%pages %logo
I renamed my logo to what you suggested my-logo.jpg and put that name in the show image area
I deleted all files in the header folder
I put my-logo.jpg in the images folder
I'm still getting 2 headers ouryogablog.com
Help please

I see no images at all. Could you attach an export of your settings to a reply? Do you have a caching plugin installed?
