Customized comment form with CForm Plugin

Hi,

I am using the Atahualpa 3.4.5 the latest version of the theme. I have been regualrly updating the version. I must accept this is one the best I have ever found. Thanks to guy who developed and is/are supporting this theme.

I need help, I am not very techie type.

I need to customize the comment forms in posts & pages, I am using Cform V 11.3 latest version. It gives an option to use the comment forms which is pre-configured. However, the customization needs to be done at comments.php in Atahualpa 3.4.5. They have given some instruction as to how to change some code but it is not full. I did tried myself to change the codes but somehow the forms didn't appear with the all feilds. Only the submit button is shown.

There is link which explain how to do it, but I am unable to comprehend.

Is it possible to get help with detailed instructtions.
