Instal BMI Calculator

I have a BMI calculator that I would like to install into my blog. It's currently filed in a zip file on my computer. So far, I've created an empty php link on one of the sidebars and would like it to appear there.

How can I upload and install it to the blog in the first place?

I thought about using 'Manage Plugins <Add new' but am not certain of the effect on the blog itself.

Would be very grateful for some ideas.

OSX 10.11.5 WP 4.x Atahualpa(all) Safari, Firefox, Chrome
is it a plugin? a zip file is compressed. you need to unzip it then upload it to your web site (using an ftp program) then follow the instructions of the program to install it
