There will be no more development for Atahualpa (or any other theme), and no support. Also no new registrations. I turned off the donation system. I may turn the forum to read only if it gets abused for spam. Unfortunately I have no time for the forum or the themes. How to add an email form in a page on a static site

Simple as that. Which would be the procedure to show an email form on the "Contact Us" page?

OSX 10.11.5 WP 4.x Atahualpa(all) Safari, Firefox, Chrome
why don't you use one of the contact form plugins

Contact Form 7 works fine for me on 2.9.2. If you're using 3.0 you may want to search it from your WP Dash to see if there's a newer version available.
