How do I prevent my opt-in form fields from auto-expanding?

Hello,

I want an opt-in form in the body of one page only. I paste the aweber provided code in the html section of the wordpress editor and my page displays correctly in wordpress visual mode with the name and email input boxes the correct size as shown here:

But when I preview the page to see what it will look like before I publish it, the name and email boxes extend all the way across the center column of the page and are out of proportion, as shown here:

I have tried to change the size of the boxes in the raw html but wasn't successful.
I have tried to use the javascript code instead of the raw html code for the optin form but that didn't work either.

Is my code in the wrong place?
Are the size of the name and email boxes auto-expanding due to some other setting?

Wordpress Version 3.0.5
Altuahalpa Version 3.5.3

NOTE: I don't want the form in a sidebar widget, I know how to do that. I want the form in the body content of a single page. Thanks!

Thank you! Pics attached.
