I searched for and could not find the answer (doesn't mean it wasn't there).

I changed the header from live text of the name of the site to a jpeg of the logo.
I made that change through WP->Appearance -> Montezuma options-> Sub templates ->header.php

Yes.
If I update to the new version will I have to re-do it.

anything you enter into one of the theme options is stored in the database and will survive an update.
